Non-Surgical Treatments for Periodontal Disease

The first line of defense when treating gum disease is a deep cleaning technique called scaling and root planing. The goal of scaling and root planing is to remove existing tartar, plaque, and bacteria from the teeth, including on the root below the gum line.

Scaling involves using an ultrasonic device to remove buildup deep down where regular devices can’t reach. In the planning step, rough areas of the tooth and the root are “planed,” or smoothed out, which provides a clean, healthy surface for the gum tissue to reattach to the tooth. Scaling and root planing can help treat earlier stages of gum disease and may prevent the need for more invasive treatments.

Pocket Depth Reduction

When your mouth is in good health, the gum tissue is firm and fully surrounds the teeth, supported by the bone. Periodontal disease damages these hard and soft tissues, creating open spaces around the teeth called pockets. As the pockets grow, bacteria become much easier to collect inside, leading to increased damage over time. If these supportive structures continue to degrade, the tooth eventually falls out or needs to be extracted.

Pocket reduction, or “flap surgery,” involves folding the gum tissue back to remove bacteria, plaque, and tartar beneath the gums. Any tissue that is too damaged to restore itself is also removed. The healthy gum tissue is then stitched back into place. With diligent oral care, the gum tissue gradually re-attaches to the teeth.

Regeneration

Severe gum disease can lead to the breakdown of bone and gum tissue. In this case, the first step is to fold back the gum tissue to thoroughly clean out the bacteria, plaque, and tartar that have built up in the gum pockets. If there is significant bone deterioration, your dentist will then perform a bone graft, which helps stimulate the growth of new bone. In other cases, they may administer a biostimulatory protein that regenerates bone growth to repair the areas damaged by periodontal disease.

Soft-Tissue Graft

Periodontal disease frequently leads to gum recession (or gingival recession), in which the gums recede from the tooth structure, revealing more of the root. The teeth appear longer and have areas sensitive to heat and cold. Receding gums also expose more of the tooth, including the root, to bacteria, plaque, and tartar. A soft tissue graft restores receded gum tissue using a graft, typically taken from the roof of your mouth. Restoring the gum line helps protect the surface of the tooth from plaque and bacteria and halts tissue damage. Treating gum recession also has cosmetic benefits and is sometimes performed for aesthetic purposes.
